Qualifications

• Requires QDDP or Designated Coordinator per state/federal rules.
• Excellent written, verbal, and customer service skills.
• Strong organization, time management, and problem-solving skills.
• Ability to independently manage multiple clients, deadlines, and documentation.
• Proficiency with Microsoft applications and databases.
• Travel across the Minneapolis–St. Paul metro area.
• Valid Class D driver's license and personal vehicle insurance.
• Commitment to diversity, equity, and inclusion.
• Ability to pass criminal background check and drug/alcohol test after offer.

Responsibilities

• Develop, write, monitor, and evaluate individualized client goals and outcomes.
• Complete and maintain client files, assessments, documentation, reports, and other required paperwork.
• Meet regularly with clients and their support teams, including families, guardians, case managers, residential providers, and other service providers.
• Conduct visits to clients at their community job sites to check in on their employment, identify concerns, and help evaluate whether the job continues to be a good fit.
• Maintain professional communication with clients, their support teams, employers, and internal program staff.
• Submit waiver, funding, and rate documentation and follow up to ensure accurate Service Agreements are in place.
• Coordinate client transportation needs and other services as needed.
