Job summary

Merrick Inc. in Vadnais Heights, MN, seeks a Program Coordinator to plan, implement, and document services for assigned clients within Enhanced Services.

You will create and update Client Files, face sheets, and complete all necessary paperwork, documentation, reports, and meetings for all assigned clients. The role requires independent travel in the Minneapolis–Saint Paul area using a personal vehicle, and a flexible schedule to meet client needs.

Qualifications

• Qualification tracks include a bachelor’s degree in human services with 1 year of direct care experience, an associate degree with 2 years, a diploma with 3 years, or four years of direct care experience under qualified supervision.

Responsibilities

• Assess, develop, write, and evaluate client assessments and goals based on each client’s Support Plan Addendum.
• Measure client outcomes and take actions to facilitate achievement of outcomes.
• Complete client-specific paperwork, documentation, and reports for assigned clients.
• Schedule and conduct all required meetings for assigned clients.
• Create and update Client Files and client face sheets for all assigned clients.
• Communicate regularly with guardians, homes, family members, case managers, and program leaders.
