Job summary

Merrick seeks a Program Coordinator to plan, implement, and document services for assigned clients in Enhanced Services. You will manage client files, face sheets, and all paperwork while maintaining professional relationships with families, case managers, and staff in the Minneapolis–Saint Paul area.

Role requires independent travel in the metro area using a personal vehicle, with a flexible schedule to meet client needs. DEI commitment is emphasized.

Qualifications

• QDDP or Designated Coordinator required as defined by statute.
• Bachelor’s degree in a related field and 1 year direct care experience—including seniors or disabilities.
• Associate degree in a related field with 2 years of direct care experience.
• Diploma in a related field with 3 years of direct care experience.

Responsibilities

• Assess, develop, write, and evaluate client assessments and goals.
• Measure client outcomes and facilitate achievement of outcomes.
• Complete client paperwork, documentation, and reports.
• Schedule and conduct required meetings for clients.
• Create and update Client Files and client face sheets.
