Job summary

Merrick seeks a Program Coordinator to support adults with intellectual and developmental disabilities across the Minneapolis–Saint Paul metro area. You will help coordinate employment goals, document progress, and maintain client files.

You will visit community job sites, partner with families and case managers, and ensure accurate service agreements and transportation as needed. Commitment to DEI and strong organizational skills are essential.

Qualifications

• Must be a Qualified Developmental Disability Professional (QDDP) or Designated Coordinator as defined in federal and/or state statute.
• Excellent written, verbal, and customer service skills.
• Strong organization, time management, and problem-solving skills.
• Ability to independently manage multiple clients, deadlines, meetings, and documentation requirements.
• Ability to pass a criminal background check and drug/alcohol test following a conditional job offer.
• Ability to use Microsoft applications, databases, and internet-based systems.

Responsibilities

• Develop, write, monitor, and evaluate individualized client goals and outcomes.
• Complete and maintain client files, assessments, documentation, reports, and other required paperwork.
• Meet regularly with clients and their support teams, including families, guardians, case managers, residential providers, and other service providers.
• Conduct visits to clients at their community job sites to check in on their employment, identify concerns, and help evaluate whether the job continues to be a good fit.
• Maintain professional communication with clients, their support teams, employers, and internal program staff.
• Submit waiver, funding, and rate documentation and follow up to ensure accurate Service Agreements are in place.
• Coordinate client transportation needs and other services as needed.
