Job summary

Chesapeake Regional Healthcare is seeking a Pharmacy Medication History Technician to collect accurate home medication histories for admitted patients in the Emergency Department. You will interview patients/families, review records, and document in the EMR to guide medication orders at admission and discharge.

Qualifications

• Greater than 2 years as a Pharmacy Technician with retail or hospital experience.
• Licensed with Virginia Board of Pharmacy as a Pharmacy Technician or Intern.
• National Pharmacy Technician certification within 1 year of hire.

Responsibilities

• Collect accurate medication and allergy history from patients and informants.
• Document medication histories into the EMR using reconciliation tools.
• Verify doses and drugs, including OTCs and herbals, per policy.
• Communicate with nurses, pharmacists and other staff to resolve discrepancies.
• Assist with pharmacy-related duties in ED and inpatient units as needed.
• Participate in quality improvement and attend departmental communications daily.
• Maintain knowledge of high-alert medications and pharmacology references.

Education

High School diploma or equivalent
