Job summary

Northwell Health in New York is seeking a Registered Nurse to perform an age-specific plan of care using the nursing process across a designated patient group. You will coordinate with physicians and the health care team to deliver safe, effective care and direct activities on the patient care unit.

Responsibilities include assessing physiologic and psychosocial health, formulating care plans, implementing treatments, educating patients and families, and collaborating for discharge planning.

Qualifications

• Graduate from an accredited School of Nursing.
• Active NYS RN license is required.
• BLS required; specialized certifications as needed.

Responsibilities

• Assesses patient’s physiologic health status.
• Assesses patient’s psychosocial health status.
• Identifies patient outcomes and goals.
• Formulates and prioritizes the plan of care.
• Implements plan of care and monitors safety.
• Evaluates care and outcomes.
• Collaborates with care team and physicians.
• Performs skin care and decubitus procedures (Grade I-IV).
• Administers medications safely and accurately.
• Performs patient and family education.
• Performs related duties as required.
