Job summary

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing required.
• Bachelor’s Degree in Nursing preferred.
• Current NYS/CT RN license and BLS.

Responsibilities

• Assesses the patient’s physiologic health status and documents findings.
• Assesses the patient’s psychosocial health status and coping needs.
• Identifies patient outcomes and writes measurable goals.
• Plans and prioritizes care with the care team.
• Administers medications safely and documents administration.
• Educates patients and families about care plans and discharge.
• Collaborates with physicians and health team members in treatment and planning.
• Participates in discharge planning and referrals.
