Wellstar Health System is seeking a Medication History Specialist to join the Emergency Department team. You will obtain and document the best possible medication history from patients identified for admission, reducing duplications and errors.

This role requires excellent communication, professionalism, and ability to work with nurses, physicians, and pharmacists under the direction of pharmacy leadership and ED leadership.

Formación

• High School Diploma or GED required.
• CPHT or PHT certification required.
• Minimum 2 years pharmacy technician experience; retail experience preferred.

Responsabilidades

• Medication Reconciliation: obtain and clarify home medication list.
• Interact with nurses, physicians, pharmacists and other medical professionals as appropriate.
• Provide customer service during phone calls and patient/family interactions.
• Communicate effectively with all patient populations to gather histories.
• Communicate with pharmacists, nurses, family members and others outside the hospital to gather information.
• Demonstrate professionalism and interpersonal skills.
• Make rounds in the Emergency Department on the nursing staff, physician staff and patients.
• Performs other duties as assigned.
• Complies with Wellstar Health System policies and code of conduct.
