Wellstar Health System is seeking a Medication History Specialist to join the Emergency Department team. The role focuses on obtaining and documenting patients' home medication histories to support safe hospital treatment and discharge planning.

Reporting to Pharmacy Leadership and ED Leadership, the MHS must pass competency tests and enter data accurately into the electronic medical record while communicating with patients, families and health care providers.

Qualifications

• High School Diploma or GED required.
• CPHT Pharmacy Technician Certification or PHT Registration required.
• Minimum 2 years pharmacy technician experience, retail preferred.
• Strong computer and typing skills; good communication and interpersonal abilities.

Responsabilités

• Medication Reconciliation: obtains and clarifies home medication list with patients/families and resources.
• Customer Service: provides professional interactions with patients, families and staff.
• Make rounds in ED and coordinate with nurses and physicians; resolve discrepancies; adhere to policies.
