Responsibilities

• Obtain and document the best possible medication history for new admissions.
• Enter home medications into the electronic medical record accurately.
• Collaborate with physicians, pharmacists, and nurses to resolve discrepancies.
• Maintain patient privacy and demonstrate compassion at the bedside.
