Wellstar Health System, Inc. is seeking a Medication History Specialist to obtain and document comprehensive home medication histories for patients admitted through the Emergency Department.

You will use phone and in-person interviewing techniques, review prescription bottles and medical records, and coordinate with nurses, physicians, and pharmacists to ensure accurate medication orders. You must have pharmacy technician experience (minimum 2 years), pass the medication reconciliation

Qualifikationen

• High School Diploma or GED required.
• Minimum 2 years pharmacy technician experience; retail experience preferred.
• CPHT and PHT certifications required upon hire.
• Ability to lift up to 30 pounds; basic typing and computer skills required.
• Excellent customer service and communication skills; ability to work in a team.

Aufgaben

• Obtain and clarify patients' home medication lists using resources and patient/family interviews.
• Enter medication history into the electronic medical record with accuracy.
• Coordinate with nurses, physicians, pharmacists and other staff for medication history information.
• Address and resolve discrepancies in home medications and communicate findings.
• Conduct rounds or rounds-like checks with ED staff to ensure up-to-date medication histories.
