Job summary

Saint Francis Hospital and Medical Center in Hartford, CT seeks a Nurse Senior – Navigator (Complex Care Coordinator) to support emergent and high-risk patients in the Emergency Department. The RN collaborates with the ED team to deliver coordinated, patient-centered care and smooth transitions of care.

The role requires ED/inpatient nursing experience, CT RN licensure, and a BSN preferred. Part-time 24 hours/week, day shift, with rotating weekends and flexible options.

Qualifications

• Bachelor's degree in Nursing (BSN) strongly preferred and generally expected for successful candidates.
• RN license valid in Connecticut.
• Minimum of three years of Emergency Department and/or acute inpatient nursing experience with complex patient populations.

Responsibilities

• Perform real-time assessments of medical, psychosocial, and behavioral health needs in the ED.
• Collaborate with ED physicians, APPs, nurses, case management, and other teams to plan care for high-risk patients.
• Coordinate referrals and connect patients to inpatient services, specialty providers, and community resources to ensure safe transitions of care.
