Saint Francis Hospital in Connecticut seeks an Emergency Department RN Case Manager to coordinate patient care in a busy ED, focusing on care coordination, utilization management, and safe transitions of care.

Collaborate with ED physicians and nurses, identify appropriate levels of care, and educate patients about plans and resources. CT RN license required; Bachelor’s in Nursing and 3+ years ED experience preferred. BLS certification required; CCM preferred within designated timeframe.

Competenze

• Graduation from an accredited nursing school.
• Valid RN License in Connecticut.
• Bachelor’s degree in Nursing (preferred).
• Minimum of three (3) years of acute care RN experience (preferred).
• Emergency Department experience strongly preferred.
• Knowledge of utilization management, level-of-care determination & care coordination.
• Familiarity with CMS/HCFA, OBRA, CT regulations, and Joint Commission standards.

Mansioni

• Coordinate care across ED, inpatient, observation, and post-acute settings.
• Identify appropriate levels of care and facilitate admissions, transfers, or discharges.
• Collaborate with ED providers and interdisciplinary teams to assess patient needs.
• Support patient flow, throughput, and length-of-stay initiatives.
• Educate patients and families on care plans, resources, and next steps.
