Job summary

Responsibilities

• Collaborate with ED providers and care teams to assess patient needs at presentation.
• Identify appropriate levels of care and facilitate admissions, transfers, or safe discharges.
• Coordinate care across inpatient, observation, and post-acute settings.
• Support patient flow, throughput, and length-of-stay initiatives.
• Conduct clinical, psychosocial, and discharge planning assessments.
• Apply nationally recognized medical necessity and utilization criteria.
• Ensure compliance with regulatory, accreditation, and payer requirements.
• Educate patients and families on care plans, resources, and next steps.
