Trinity Health Of New England’s Saint Francis Hospital and Medical Center seeks a Nurse Senior - Navigator (Complex Care Coordinator) to support high-risk ED patients with complex medical, social, and behavioral needs. Employment Type: Part time Shift: Day Shift Description: Position Purpose In the Emergency Department (ED) at Saint Francis Hospital and Medical Center, the Nurse Senior - Navigator, informally known as the Complex Care Coordinator, plays a crucial role in supporting emergent and high-risk patients with complex medical, social, and/or behavioral health needs. The Registered Nurse (RN) in this role collaborates closely with the ED care team to deliver coordinated, patient-centered care and support safe, effective transitions across the continuum.
Part-Time: 24 hours per week - Day Shift Variable shift options aligned with departmental needs Rotating weekends, flexible.
This is a salaried position with a range of $83,304 to $133,328 (based on 40 hours/week).
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
The pay range may also vary within the stated range based on location.
