Job summary

Trinity Health Of New England’s Saint Francis Hospital and Medical Center seeks a Nurse Senior - Navigator (Complex Care Coordinator) to support high-risk ED patients with complex medical, social, and behavioral needs. The RN collaborates with the ED team to ensure coordinated, patient-centered care and safe transitions across the continuum.

The role requires a BSN (preferred) and CT RN licensure, with a minimum of three years in ED or acute inpatient care.

Qualifications

• BSN strongly preferred; RN license required in CT.
• Minimum of 3 years of Emergency Department and/or acute inpatient nursing experience.

Responsibilities

• Conduct comprehensive, real-time assessments of patients in the ED.
• Collaborate with ED physicians, APNs, nursing staff, case management and ancillary teams.
• Coordinate referrals to inpatient services, specialists, and community resources to support safe dispositions.
