Job summary

Corewell Health in Big Rapids, MI is seeking a Community Health Worker to provide peer support, education on wellness and self-management, and guidance for accessing community resources. You will conduct home visits, help coordinate care, and support families toward healthier outcomes.

The role emphasizes cultural competency, collaboration with care teams, and accurate documentation of assessments, plans, and program data.

Qualifications

• Required High School Diploma or equivalent and successful completion of paraprofessional CHW Training Program (Completion within 180 days from hire date)
• BLS certification within required timeframes (60–90 days) per ARC/US regulations
• Driver's license and eligible to drive to appointments

Responsibilities

• Participates in community events to promote health maintenance and disease prevention
• Conducts home visits to assess client and family needs
• Develops and implements care plans, monitors progress and evaluates goals
• Coordinates with clients, families and providers to ensure continuity of care
• Assists clients in accessing community resources and arranging appointments
• Mentors and advocates for clients to increase independence and skills
