Job summary

Qualifications

• One year’s work experience in a community-based, healthcare, or health and human services setting.
• Fluency in Spanish is desired.

Responsibilities

• Conducts initial assessments for new participants, focusing on high-risk populations.
• Maintains trust with participants and provides referrals to services.
• Provides care coordination and linkage to community resources.
• Documents records accurately and collects data.
