Job summary

Corewell Health in Southfield, MI is seeking a Community Health Worker to provide peer support and wellness education through home visits, calls, and community outreach. You will help clients access resources and coordinate care plans while advocating for independence.

Qualifications

• High School Diploma or equivalent and completion of paraprofessional CHW Training Program.
• BLS certification within 60–90 days of hire.
• Valid driver’s license and reliable transportation.
• Associate’s degree or equivalent.
• At least 3 years of relevant work experience.
• Knowledge of community resources and social/health services.

Responsibilities

• Participates in community events and activities to promote health maintenance and disease prevention.
• Conducts home visits to assess client and family needs.
• Assists with care plan development and program services.
• Follows up with clients and families to evaluate services and plan for future needs.
• Assists with scheduling medical appointments and coordinating access to care.
• Aids clients in accessing community resources such as food, clothing, shelter, and medical insurance.
• Mentors and advocates to help clients increase independence and skills.
• Refers clients to appropriate service agencies for social, emotional, and other challenges.
• Collaborates with clients, families, and professionals to coordinate services and support.
• Documents assessments, plans, evaluations, and program data.
