Qualifications

• Two years of experience in pharmacy practice. Months spent in an ASHP-accredited residency may substitute for experience.

Responsibilities

• Ensure appropriate drug therapy and nutrition, including indication, dose, interactions, and allergies.
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse events.
• Communicate pharmacotherapeutic recommendations using medical records and documentation systems.
• Provide drug information and education to medical teams, nursing, and dietitian staff.
• Provide patient/family counseling and reconcile medication lists at visits.
• Provide primary preceptorship of residents and student pharmacists.
• Perform duties of a Pharmacist 2.
