Job summary

Ennoble Care seeks an Embedded Care Coordinator to join our on-site team in the Atlanta, Georgia area, focusing on Sandy Springs. You will build relationships with residents and families, coordinate care using our EMR, and work closely with providers within long-term care facilities to ensure continuity of care.

The role emphasizes proactive on-site care, RPM integration, and close collaboration with facility staff in a family-practice style model.

Qualifications

• Legally authorized to work in the United States.
• Active, unrestricted clinical certification or licensure (MA/CMA, LPN, RN).
• Minimum 1 year of patient-facing experience in long-term care or similar clinical setting.
• Proficiency with Microsoft Teams, email, phone and in-person communication.
• Willingness to work on-site in Atlanta, GA, with travel between facilities if required.

Responsibilities

• Coordinate care for long-term care residents using Ennoble Care’s EMR.
• Maintain regular, proactive on-site engagement to support care plans and transitions.
• Collaborate with Ennoble Care providers within the long-term care setting.
• Serve as liaison between providers, facility staff, residents and families.
• Document patient interactions and care coordination activities accurately.
