Job summary

Ennoble Care is seeking a full-time Care Coordinator, Embedded to support on-site care at partnered long-term care facilities in the Atlanta, GA area, with a focus on Sandy Springs. You will work closely with providers in a family-practice style model emphasizing continuity and patient-centered support.

This role requires active clinical licensure (RN/LPN/CMA/MA) and at least 1 year of patient-facing LT care experience.

Qualifications

• Active clinical certification or licensure (RN/LPN/CMA/MA).
• Minimum 1 year of patient-facing LT care or similar clinical setting.
• Legally authorized to work in the United States of America.

Responsibilities

• Coordinate care for long-term care residents using EMR.
• Engage on-site with residents to support care plans and transitions.
• Collaborate with Ennoble Care providers in the LT care setting.
• Serve as liaison between providers, facility staff, residents, and families.
• Communicate patient needs and changes to the clinical team promptly.
• Document patient interactions and care coordination activities.
• Follow established workflows and escalation protocols.
• Contribute to team collaboration and facility partnerships.
