Circadia Health, a growth-stage healthcare AI company, seeks a Nurse Practitioner to join on-site at Sunny Hills in Fullerton, CA. You will see residents first, interpret AI‑driven health signals in real time, and coach facility teams to implement timely interventions that prevent readmissions.

In this full‑time role, you’ll conduct initial and follow‑up assessments, train staff on Circadia workflows, document findings, and collaborate with clinical ops and product teams to improve patient care

Qualifications

CA NP license (ANCC or equivalent) in Adult/Family/Geriatric/Acute.
Eligible for Medicare participation; company bills on your behalf.
2+ years NP experience with complex adult populations; independent practice.
Valid CA driver's license; willing to travel locally.
Thrives in a dynamic startup environment.
English required

Responsibilities

Direct patient care: conduct initial and follow-up assessments; adjust plans; coordinate with providers.
Tech & bedside integration: train staff and embed Circadia into workflows.
Data-to-action: interpret AI signals and prioritize high-risk patients for timely interventions.
Documentation: complete notes within 24 hours of each encounter.
Mentor & collaborate: support NP onboarding and partner with clinical ops/product teams.
High-volume rounding: conduct visits at partner SNFs with 30+ per day as applicable.
