Job summary

Circadia Health is seeking a Nurse Practitioner to join us on-site at Sunny Hills in Fullerton, CA. This embedded role blends direct patient care with tech‑to‑bedside integration, coaching facility teams to apply predictive insights in real time.

You’ll perform assessments, initiate and adjust plans of care, document within AI‑assisted workflows, and mentor NP onboarding while aiming to reduce readmissions and improve post‑acute care.

Qualifications

• CA NP license required.
• Eligible for Medicare participation.
• 2+ years NP experience with complex adult populations.

Responsibilities

• Direct patient care: initial and follow-up assessments; plan of care adjustments.
• Tech → bedside integration: train staff and standardize Circadia into workflows.
• Data into action: interpret trends and prioritize high-risk patients for timely interventions.
• Documentation: complete notes within the same day using AI-powered tools.
• Mentor & collaborate: support NP onboarding and work with clinical ops/product teams.
