Ennoble Care is seeking a full-time Embedded Care Coordinator for Warrenton, VA, working on-site within partnered long-term care facilities. You will coordinate patient care, engage residents and families, and collaborate with clinicians to ensure smooth care transitions in a family-practice style model.

The role emphasizes RPM, proactive care coordination, and strong relationships with residents and staff.

Qualifications

• Active, unrestricted clinical certification or licensure is required.
• Minimum 1 year of patient-facing experience in long-term care or similar clinical setting.
• Proficiency with Microsoft 365 including Outlook, Word, Excel.
• Ability to work full-time on-site in Warrenton/Reston area.

Responsibilities

• Coordinate care for long-term care residents using Ennoble Care’s EMR.
• Maintain regular on-site engagement to support care plans and transitions.
• Collaborate with Ennoble Care providers within the facility setting.
• Serve as liaison between providers, facility staff, residents, and families.
• Communicate patient needs and changes to the clinical team promptly.
• Document patient interactions and care coordination activities.
• Follow established workflows and escalation protocols.
• Contribute to team collaboration and facility partnerships.
