Job summary

Ennoble Care in Warrenton, VA is seeking a full-time Embedded Care Coordinator. You will be onsite in partnered long-term care facilities, building trusted relationships with residents and families, coordinating care, and supporting care plans in collaboration with our providers.

The role emphasizes on-site presence, RPM integration, and proactive care coordination within a family-practice style team, with Monday–Friday hours and no weekends or on-call.

Qualifications

Active, unrestricted clinical certification or licensure.
Minimum 1 year patient-facing experience in a long-term care setting.
Proficient with Microsoft 365 and healthcare terminology.
Reliable, proactive communication with residents and families.

Responsibilities

Coordinate care for long-term care residents using the EMR.
Maintain on-site engagement to support care plans and transitions.
Collaborate with Ennoble Care providers within the facility.
Liaise between providers, facility staff, residents and families.
Document patient interactions and care coordination activities.
Follow workflows and escalation protocols.
Support strong facility partnerships.
