Job summary

Ennoble Care LLC seeks a full-time Embedded Care Coordinator to join our on-site team at partnered long-term care facilities in Leesburg, VA. You will work closely with providers to deliver patient-centered care in a family-practice style model, with emphasis on continuity and RPM integration.

You will coordinate care, document activities, and maintain strong relationships with residents and families while adhering to established workflows and on-site schedules. Equal opportunity employer.

Qualifications

• Legally authorized to work in the United States.
• Active, unrestricted clinical certification or licensure.
• Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting.
• Ability to work on-site in Reston/Leesburg area, 8:00 a.m. to 4:30 p.m. EST.

Responsibilities

• Coordinate care for long-term care residents using Ennoble Care’s EMR.
• Maintain regular, proactive on-site engagement with residents to support care plans and transitions.
• Collaborate with Ennoble Care providers in the long-term care setting.
• Serve as a liaison between providers, facility staff, residents, and families.
• Document patient interactions and care coordination activities.
• Follow established workflows and escalation protocols.
• Contribute to team collaboration and facility partnerships.
