Job summary

Care Lync is seeking an Eligibility Specialist to provide case management and service coordination for adults with disabilities, assisting them to live independently in their home and community. The role develops individualized care plans, coordinates services, and monitors outcomes to ensure professional, comprehensive care.

The position requires a BS/BA in a related field, proficiency with Microsoft Office and state databases, and CPR/First Aid certification with state clearances.

Qualifications

• BS/BA degree in related course work.
• Experience with Microsoft Office and state-maintained databases.
• Maintain CPR/First Aid certification and updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage.
• Please specify if you are bi-lingual (English-Spanish).

Responsibilities

• Complete assessments to identify participant needs and desires for service delivery.
• Develop a Person-Centered Service Plan for each participant and update annually or with significant changes.
• Ensure the participant choice of providers by providing information for informed decisions.
• Ensure the Behavioral Support Plan and Crisis Plan align with the ISP.
• Coordinate with other care coordinators to ensure seamless delivery of services.
• Research and develop alternative solutions to participant needs.
• Maintain confidentiality and complete required trainings.
• 50% travel to participants as needed.
