Job summary

Samish Indian Nation in Washington State is seeking an Elders Wellness Support Specialist to help older adults stay healthy, safe, and independent by delivering comprehensive assessments, developing individualized service plans, and providing case management within the Elders Department.

You will coordinate resources, advocate for participants, conduct wellness checks and home visits, support health education programs, document activities with HIPAA compliance, and collaborate with tribal

Qualifications

• Associate's degree or 3 years related experience.
• Demonstrate exceptional customer service and client relation skills.
• Strong time management, organization, and communication skills.
• Detail oriented, proactive problem solver.
• Intermediate Microsoft 365 skills with ability to learn reporting software.
• First Aid, CPR and AED certifications within 30 days of hire.
• TB test within 12 months prior to employment; valid driver's license and tribal vehicle operation.

Responsibilities

• Provide services and support to Elders to promote health and independence.
• Conduct comprehensive assessments and develop individualized service plans.
• Deliver case management and coordinate resources and referrals.
• Advocate for participants and monitor progress toward goals.
• Conduct wellness checks and home visits; participate in health events.
• Maintain confidentiality and HIPAA compliance; document accurately.
