Samish Indian Nation in Washington State is seeking an Elders Wellness Support Specialist to help older adults stay healthy, safe, and independent by delivering comprehensive assessments, developing individualized service plans, and providing case management within the Elders Department.

Qualifications

Description

The Elders Wellness Support Specialist provides participants in the Elders Department with a wide range of services and support designed to help them remain healthy, safe, and independent within the community. This position is responsible for delivering culturally appropriate services, conducting comprehensive assessments, developing individualized service plans, providing case management, coordinating resources, advocating for participants, and monitoring progress toward identified goals. The Elders Wellness Support Specialist works collaboratively as a member of the Elders Department team, contributing to a positive work environment, supporting department goals, and demonstrating commitment to serving person-centered support.

  • Works collaboratively with Samish departments, local agencies, and community partners to ensure participants have access to needed resources, services, and support.
  • Conducts comprehensive assessments of participants' needs, determines program eligibility, provides case management, and develops individualized service plans to address identified needs and goals.
  • Conducts wellness checks and home visits to evaluate the safety, health, and overall well-being of participants.
  • Plans, coordinates, and participates in Samish Citizen Health Fairs, events, and other community events as needed.
  • Attends program trainings, webinars, and conferences as needed.
  • Maintains accurate organized files, including intake forms and reports; compiles data for statistical purposes and required reporting.
  • Prepares department information packets, mailings, newsletter, and website narratives.
  • Maintains strict confidentiality and HIPAA compliance regarding all client information.
  • Understands and complies with Adult Protective Services (APS) mandated reporting requirements and partners with the Samish Social Services Department and Washington state APS regarding vulnerable adult abuse reporting and crime victim advocacy.
  • Performs other duties as assigned.
Required Qualifications
  • Associate's degree in health, wellness, or human services related fields or 3 years of experience working in a field related to health, wellness, or human services.
  • Must have the ability to demonstrate exceptional customer service and client relation skills.
  • Must have strong time management, organization, and communication skills.
  • Must be detail oriented, naturally inquisitive and a proactive problem solver.
  • Must possess intermediate Microsoft 365 skills with the ability to learn data reporting software applications.
  • Excellent problem-solving and listening skills.
  • Support and contribute to a collaborative and respectful work environment that promotes teamwork.
  • Must obtain First Aid, CPR and AED certifications within 30 days of hire.
  • Must have proof of a Tuberculosis (TB) test taken within 12 months prior to initial employment date and be free from communicable disease.
  • Must possess a valid driver's license and insurance and be able to operate tribal vehicles and a chair lift.
Working Conditions

Physical Demands: Long periods of sitting (70% of time); occasional bending, crouching, stooping, stretching, reaching, lifting, carrying, and pushing (10% of time); and occasional standing and walking (20% of time). Occasionally lifts/carries up to 30 lbs and assists participants with mobility devices such as walkers and wheelchairs, requiring pushing and lifting. Must possess manual dexterity sufficient to use office equipment with frequent repetitive motion such as writing, typing, and phone use. Must possess the ability to view print on a computer screen and/or paper document; the ability to speak and hear exchanging information with end users by telephone. Mental/Visual Demands: Frequent mental and/or visual attention to task is required. Occasionally work requires heavy concentration. Frequent interruptions. Tasks involve eye-hand coordination. Working Environment: Work environment is mainly indoors with heat and some air conditioning. Must sit or stand at desk for majority of shift. Occasionally works alone without benefit of supervisor or co-worker. Unavoidable hazards/risks: Work requires minimal risks which require safety precautions (using some cleaning chemicals for light cleaning). Protective clothing or equipment may be required (goggles, gloves, masks, boots, etc.).
