Ketchikan Indian Community seeks an Adult Case Manager for the Elders Program to coordinate care and advocate for services respecting tribal values. You will conduct assessments, develop care plans, and guide caregivers to promote independence and dignity within a culturally informed framework.

This role works with an interdisciplinary team to ensure high-quality, respectful elder care, including home visits and community partnerships, while maintaining strict privacy and compliance standards.

Qualifications

Bachelor’s degree required in Social Work or related field.
Two years of experience in human services or related field.
Two years’ experience in supervisory role; CCM/ACM preferred.
Heartsaver First Aid CPR within 6 months of hire.
Valid driver’s license with acceptable driving record.

Responsabilités

Conduct thorough evaluations of elders’ physical, mental, and psychosocial needs with staff to determine care level.
Develop, implement, and adjust individualized care plans based on elder progress.
Coordinate with housing, medical, social, cultural, and vocational services to address barriers to care.
Regularly communicate through home visits, clinical check-ins, or meetings to ensure rights and needs are met.
Advocate and liaise with families, medical professionals, and social service agencies to align care plans.
Maintain accurate records and comply with privacy standards (HIPAA, etc.).
