Job summary

Qualifications

• Education as listed: high school diploma or GED with related experience in social work or community services.
• Knowledge of data entry, reporting, and basic databases is preferred.
• Excellent communication and interpersonal skills for elder outreach.

Responsibilities

• Monitor and enter service unit data into SAMS/WellSky and OAAPS for compliance.
• Develop forms to track services and supports for clients.
• Enter new client information and maintain hard copy client files.
• Prepare monthly service data reports for state and federal reporting and reimbursements.
• Coordinate initial assessments and annual re-assessments for clients.
• Document client encounters and maintain confidential files with care plans.
