Job summary

Pueblo of San Felipe is seeking an Elderly Services Outreach Specialist to introduce elders to program services, capture needs, and support care planning in the community. The role emphasizes data entry, reporting, and coordination with tribal staff and external agencies.

The position requires excellent communication, translation support in Keres when needed, and the ability to conduct outreach and educational activities for elders 50+. Evening or weekend meetings may be required.

Qualifications

• High School diploma or GED is required, with related social services experience preferred.

Responsibilities

• Monitor and enter service data into SAMS/WellSky and OAAPS databases.
• Develop forms to improve data tracking for elder services.
• Enter client information and maintain hard copy and electronic records.
• Prepare monthly reports on service units for state and federal reporting.
• Coordinate intake, assessments, and care plans for new and existing clients.
• Maintain confidential client files and respond to data access requests.
• Conduct community outreach and plan events to promote the Elderly Services Program.
• Create outreach materials and manage program communications (flyers, newsletters).
