Job summary

Harford County, Maryland seeks a Community Resources Coordinator to implement advocacy and service programs for the elderly and adults with disabilities, including Senior Care, Ombudsman, Family Caregiver Support, and MAP programs.

This professional social casework role requires strong computing skills (MS Word), excellent communication, and the ability to work some evenings/weekends in Harford County. A valid MD driver’s license is required.

Qualifications

• Bachelor's degree or higher in social work, gerontology, sociology, psychology, nursing, counseling, or a human services-related field.
• Two (2) years of experience in social casework, counseling, or aging services.
• Valid Maryland driver’s license with no more than three points.
• Must comply with safety and county policies.

Responsibilities

• Provide information to seniors, disabled individuals, and caregivers about community and institutional services.
• Complete assessments and referrals to service agencies.
• Assist clients in completing forms and follow up to ensure services are obtained.
• Documen­tation in client files in a timely manner.
• Provide case management and community education to seniors and caregivers.
