Qualifications

• Bachelor's degree in social work or related field.
• Two years of social casework or aging services experience; Master’s degree substitutes one year.
• Must possess a valid Class C non-commercial driver’s license (points ≤3).
• Must comply with safety regulations (OSHA/MOSH).
• Ability to work evenings/weekends as required.

Responsibilities

• Provide information to seniors, disabled individuals, and caregivers about community/institutional services.
• Complete assessments of client and caregiver needs; make appropriate referrals.
• Assist clients in completing applications and securing services; ensure timely documentation.
• Document client files accurately and on time; report as required.
• Deliver community education to senior and caregiver organizations; support case management.
