Job summary

WelbeHealth is seeking a Social Work Manager to lead the Social Work team and oversee social services programs for frail and elderly participants. The role collaborates across WelbeHealth's leadership and IDT to ensure high-quality care and strong participant support.

The position emphasizes direct relationships with participants, staff development, and program effectiveness, with a comprehensive benefits package and base plus bonus compensation.

Qualifications

• LSW required.
• Minimum of one (1) year of experience with a frail or elderly population.
• Valid state driver’s license with a clean DMV record.
• Copy of recent vehicle insurance required.
• CPR and First Aid certification within 180 days of hire and maintained thereafter.

Responsibilities

• Manage Social Workers and Community Health Workers including hiring, training, supervising, and coaching.
• Oversee the effectiveness of Community Health Workers and IDT participation.
• Ensure delivery of social work interventions per participant care plans and urgent needs.
• Ensure compliance in documentation, assessment, care plan completion, review, and rights/grievance processes.
• Oversee programming with Behavioral Health Specialist including support groups and counseling.
• Participate in end-of-life care coordination and home visits as needed.
