Eskaton is seeking a Social Services Coordinator to connect residents with supportive services and manage casework to help seniors and disabled residents live independently. You will coordinate with community agencies, educate residents on services, and advocate for their rights.

The ideal candidate holds a Bachelor of Social Work or related degree and at least two years of progressively responsible management experience.

Competenze

• Bachelor of Social Work or degree in Gerontology, Psychology, or Counseling is preferable; a college degree is acceptable.
• Two years of experience with progressive responsibility in management functions.
• Knowledge of supportive services and resources for senior citizens and disabled residents.

Mansioni

• Provide general case management and referral services to residents needing assistance.
• Establish linkages with community agencies and service providers; help determine best pricing and flexible services.
• Set up a directory of providers for staff and residents.
• Refer residents to service providers in the general community and advocate as needed.
• Educate residents on service availability, procedures, and client rights; provide advocacy.
• Monitor service provision and maintain confidential records; update progress with providers.
• Help residents build informal support networks and coordinate volunteer programs.
