Job summary

Center for Elders’ Independence (CEI) seeks a Patient Care Navigator to serve as a generalist within the HCBA department. You will be the first point of contact, coordinating administrative tasks, scheduling, and communication with participants and SNFs.

This role supports care coordination in a high-volume environment. Requires a background in health plan operations or related clerical experience, strong customer service, and proficiency with PCs.

Qualifications

• Associate degree preferred with 3-5 years of health plan operations experience preferred.
• Strong customer service and oral communication skills required.
• Health plan experience with member coordination/communication and clerical management preferred (3-5 years).
• Basic knowledge of PCs and related software required; accuracy and dependability essential.

Responsibilities

• Acts as initial point of contact for department calls and triages calls appropriately.
• Provides administrative support for non–PACE care coordination functions (printing/faxing/scanning participant documents).
• Orders office supplies for HCBA and manages emergency alert system installation.
• Initiates calls for clinical or administrative information as requested.
• Receives information and forwards to correct recipient; coordinates discharge transport and follow-up appointments.
