Job summary

The Eastern Nebraska Office on Aging (ENOA) seeks a Services Coordinator to determine and monitor long-term care needs for elders eligible for HCBS, coordinating services with participants and supports to promote safe, independent living.

Responsibilities include comprehensive assessments, developing participant-centered plans, coordinating with providers, monitoring outcomes, and advocating for clients while protecting confidentiality under HIPAA.

Qualifications

  • Bachelor’s degree in human services, health/medical field or related area is required.
  • Registered Nurse licensed in Nebraska preferred or required as stated.
  • Experience in long-term care, gerontology or case management is desired.

Responsibilities

  • Perform comprehensive assessments to identify strengths and needs.
  • Develop participant-centered plans for services and supports.
  • Coordinate home and community based services with providers and informal supports.
  • Monitor service delivery and outcomes, ensuring timeliness and quality.
  • Advocate for client autonomy while maintaining HIPAA confidentiality.
  • Collaborate with the care team and communicate clearly with participants and families.
