The Center for Elders’ Independence in Oakland, CA seeks a Patient Care Navigator to serve as the HCBA department’s generalist and first point of contact. The role supports care coordination, triages calls, and facilitates appointments for participants.

You will work with an interdisciplinary team, handle information flow, arrange discharge transport, and help ensure timely, high-quality care within CEI’s integrated services model.

Formación

• Associate degree preferred and 3–5 years of experience in health plan operations or related field.
• Excellent customer service and verbal communication skills are required.
• Health plan experience with member coordination/communication and clerical management is preferred (3–5 years).
• Basic knowledge of PCs and related software; accuracy and attendance are critical.

Responsabilidades

• Serve as the initial point of contact for department calls and triage accordingly.
• Provide admin support for non–PACE care coordination tasks (printing, faxing, scanning participant documents).
• Order office supplies for HCBA and manage emergency alert system installation for participants.
• Coordinate discharge transport and schedule internal and external appointments.
• Receive information, ensure delivery to the correct recipient, and facilitate communication with SNF and case managers.
• Document participant concerns and facilitate resolution; assist with data collection and scheduling.
• Support the CCSC team and care coordination services as needed.
