Job summary

Center for Elders’ Independence in Oakland, CA is seeking a Referral Coordinator I under the supervision of the Health Information and Referrals Manager. You will be the single point of contact for scheduling outside specialists, handling referrals, coordinating with clinics, and ensuring timely appointments.

The role requires accurate data entry in electronic systems, strong communication with providers and families, and the ability to work with a multidisciplinary team to support CEI’s mission

Qualifications

• Associate degree or equivalent work experience.
• Minimum one year’s experience authorizing and scheduling medical appointments.
• Experience with electronic health records (EHR).
• Proficiency with MS Office.
• Proficient English communication (oral and written).

Responsibilities

• Receives and processes referrals and coordinates with participants, caregivers and the interdisciplinary team.
• Coordinates with external specialist/vendor offices to schedule appointments and transmit necessary information.
• Maintains awareness of CEI contracted provider networks with Contracts department.
• Documents appointment dates/times and referral tracking in EHR and related systems.
• Obtains referral authorization numbers and enters them into referral systems for claims processing.
• Collaborates with Health Information Coordinator to ensure clinical information is available for external specialists.
• Ensures all arrangements (transportation, escort, interpreter) are in place for appointments.
• Demonstrates effective communication and problem-solving, escalating complex issues to HIRM when needed.
• Attends department meetings and contributes to referral process improvements.
