Job summary

Leech Lake Band Of Ojibwe seeks an Elder Advocate to support elders wishing to stay in their homes and communities. Develop individualized care plans with elders and actively involve family members to meet immediate needs, ensuring privacy and cultural sensitivity.

Responsibilities include home visits, accurate documentation, coordinating access to daily living resources, arranging transportation, and reporting to the Elder Services Director.

Qualifications

  • High school diploma or GED is required.
  • AA degree in Gerontology or related Health field preferred.
  • Four years' experience working with elders preferred.
  • CHW certification or obtain within the first year.
  • HIPAA privacy training annually.
  • Native American preference.

Responsibilities

  • During home visits, coordinate a care plan with elders and active family members.
  • Perform home visits and document each elder service/visit accurately.
  • Advocate to find resources for elders needing daily living services.
  • Coordinate transportation for enrolled elders as needed.
  • Act as a resource within the service area for elder needs and promote teamwork.
  • Provide regular reports to the Elder Services Director and maintain confidentiality.
