Job summary

Southwest Idaho Area Agency on Aging (SWIA3) is seeking a Community Health Advocate to conduct in-person home visits, coordinate caregiver resources, and provide ongoing support to families across the program service area.

The role requires independent work, strong communication, and the ability to travel throughout Idaho. Candidates should have at least a high school diploma and two years of related service delivery experience.

Qualifications

• High school degree or equivalent.
• Minimum of two years’ experience in service delivery to the disabled and aging populations preferred but required.
• Ability to work independently and remotely from team members.

Responsibilities

• Conduct home visits and determine caregiver services/resources within the project service area.
• Travel extensively to client homes and outreach destinations; out-of-town travel is required.
• Plan, coordinate, and facilitate educational events, evidence-based programs, support groups, and training opportunities.
• Provide ongoing caregiver support through care management planning and consumer-direction services.
• Act as an advocate for all clients, caregivers, and program needs.
