Job summary

Leech Lake Band Of Ojibwe seeks an Elder Advocate to support elders wishing to stay in their homes and communities. Develop individualized care plans with elders and actively involve family members to meet immediate needs, ensuring privacy and cultural sensitivity.

Responsibilities include home visits, accurate documentation, coordinating access to daily living resources, arranging transportation, and reporting to the Elder Services Director.

Qualifications

  • High school diploma or GED is required.
  • AA degree in Gerontology or related Health field preferred.
  • Four years' experience working with elders preferred.
  • CHW certification or obtain within the first year.
  • HIPAA privacy training annually.
  • Native American preference.

Responsibilities

  • During home visits, coordinate a care plan with elders and active family members.
  • Perform home visits and document each elder service/visit accurately.
  • Advocate to find resources for elders needing daily living services.
  • Coordinate transportation for enrolled elders as needed.
  • Act as a resource within the service area for elder needs and promote teamwork.
  • Provide regular reports to the Elder Services Director and maintain confidentiality.

Job description

Summary: Support elders who wish to remain living in their homes/communities. Develop plans with the elder that adequately will meet their immediate needs making certain that family members are active participants.

Education Requirements and Experience:

High School Diploma or General Education Diploma (G.E.D) is required.

A.A. Degree in Gerontology or related Health field preferred.

Four (4) years' experience working with elders preferred.

Have CHW Certification or obtain within the first year.

HIPPA compliant and data privacy training annually.

Additional Qualifications and Skills:

Understanding of the Ojibwe Culture and language in communicating with elders preferred, but not required.

Good communication and customer service skills.

Ability to identify and access the needs of the enrolled Elders.

Possess the ability to work independently, with minimal supervision, in order to ascertain individual elder needs.

Work well within a team setting and manage time effectively.

Be sensitive to the elder needs and explore all resources available.

Must possess professional integrity.

Native American Preference.

Pre-employment drug testing is mandatory for all Leech Lake Band of Ojibwe positions.

Supervisory Duties:

None

Duties/Responsibilities:

During home visits coordinate a care plan with elders, and active family member .

Perform home visits.

Accurate documentation is required for each Elder services/visit.

Advocates will assist with finding resources for the Elders in need of daily living services.

Transportation will be provided to enrolled elders as needed, when all other options have been exhausted.

Act as a resource, assist and / or access the programs, within the service area that pertain to elders needs.

Coordinate social interaction for community elders.

Provide the Director with regular reports on elder needs and activities.

Must maintain confidentiality at all times regarding clients/staff.

Assure all administrative matters are reported the Elder Services Director.

Work cooperatively as an Elder Advocate team member and promote teamwork within the Elder Care Team.

Maintain a professional attitude.

Follow GAPP policies governing staff conduct.

Attend and or conduct all mandatory meetings and trainings as required.

Maintain confidentiality and professional ethics at all times.

Travel in and out of the state on a possibly regular basis.

Understanding and sensitivity to Native American culture.

Physical Requirements:

Occasionally be able to lift up to 10 pounds.

Sedentary work with some standing while filing, copying, etc.

Must be able to decipher inclement weather.

Subject to internal working conditions.

Must be able to operate a motor vehicle safely in adverse weather conditions.
