Job summary

Hebrew SeniorLife is seeking a Supportive Services Social Worker, Outreach Specialist to coordinate care for clients served by CPEAN, including advocacy, intake, assessments and discharge planning. This on-site role requires travel across Greater Boston and Eastern MA, collaboration with referral sources, and management of SSPs with trauma-informed approaches.

The position emphasizes collaboration with multidisciplinary teams and education/outreach to prevent elder abuse.

Qualifications

• Minimum 3–5 years of experience in direct care and case management or care coordination.
• MSW required; current MA licensure required; LICSW preferred; LCSW considered if on path to LICSW.

Responsibilities

• Provide trauma-informed, person-centered coordination of care for CPEAN clients.
• Manage elder abuse prevention phone line and email, and follow up on referrals or inquiries.
• Perform intake, assessments, and discharge planning using trauma-informed tools.
• Create and track Supportive Services Plans and monitor service utilization.
• Outreach to referral sources to foster collaborative relationships and improve referral processes.
• Assess and address client housing needs with the team.
• Assist with multidisciplinary teams and elder justice task force meetings.
