Hebrew SeniorCare, Inc. is seeking a Supportive Services Social Worker, Intake Specialist for the CPEAN program in the Greater Boston area.

This on-site role coordinates trauma-informed, person-centered care for elder abuse and neglect clients, with emphasis on intake, assessment, discharge planning, and linkage to services. The position collaborates with referral sources, APS agencies, and community partners, travels to multiple sites, and contributes to trainings and policy updates.

Qualifications

• Minimum 3-5 years of direct care and case management experience; trauma survivors experience a plus.
• Master’s degree in social work; MA licensure in Massachusetts; LICSW preferred; LCSW considered.
• Knowledge of safety planning, trauma-informed and person-centered care; willingness to learn.

Responsibilities

• Provide trauma-informed, person-centered case management and intake coordination for CPEAN clients.
• Lead elder abuse prevention line and referrals; conduct intake, assessments, and discharge planning.
• Develop and track Supportive Services Plans and collaborate with community partners and APS agencies.
• Deliver trainings, assist with outreach, and participate in multidisciplinary teams and donor events.
• Travel to Greater Boston and Eastern MA sites as required.
