Hebrew SeniorLife, Inc. seeks a dedicated Supportive Services Social Worker, Intake Specialist, to coordinate trauma-informed, person-centered care for CPEAN clients. This on-site role covers Greater Boston and Eastern MA, including intake, discharge planning, and collaborative support with community partners.

You will manage the elder abuse prevention line, develop SSPs, and contribute to education, outreach, and policy improvement. Travel across Greater Boston is required.

Qualifications

• Minimum 3–5 years of experience in social work or related field.
• Master's degree in Social Work; MA licensure preferred.
• Knowledge of safety planning, trauma-informed services, and person-centered care.
• Experience creating and delivering virtual and in-person educational presentations a plus.
• Familiarity with MA Adult Protective Services system a plus.
• Willingness to travel across Greater Boston and Eastern MA.

Responsabilités

• Provide specialized case management and coordination for CPEAN clients with referring agencies.
• Lead elder abuse prevention phone line and respond to inquiries.
• Conduct intake, assessments, and discharge planning using trauma-informed tools.
• Create and track Supportive Services Plans and monitor service usage.
• Collaborate with team to provide direct supportive services as needed.
• Outreach to referral sources to foster relationships and improve procedures.
• Facilitate multidisciplinary meetings and elder justice discussions.
• Develop and deliver educational trainings on elder abuse prevention.
• Assist with documentation, policies, and post-discharge follow-up.
