Responsibilities include case management, managing the elder abuse prevention line, intake and assessment, creating SSPs, and outreach to referral sources. A MSW and MA licensure are required; LICSW preferred; travel by car is necessary.

Qualifications

• Minimum 3–5 years of experience including direct care and case management/care coordination
• Masters degree in social work and MA licensure required; LICSW preferred; LCSW considered on path to LICSW

Responsibilities

• Provide specialized case management and supportive services coordination for all CPEAN clients
• Manage elder abuse prevention phone line and email with timely follow-up
• Conduct intake, assessment, and discharge planning using trauma-informed, person-centered tools
• Create Supportive Services Plans (SSPs) and track client progress
• Lead outreach to referral sources to foster collaborative relationships
• Assess and address client housing needs
• Facilitate multidisciplinary teams and elder justice meetings
• Develop and deliver educational presentations on elder abuse prevention
• Review and enhance CPEAN documentation, policies, and protocols
• Participate in donor events and represent CPEAN in coalitions
