Hebrew Senior Care, Inc. is seeking a Supportive Services Social Worker, Outreach Specialist to coordinate trauma-informed, person-centered care for clients served by CPEAN.

The role will build and maintain relationships with community partners and referral sources across Greater Boston and Eastern Massachusetts, collaborating with Hebrew SeniorLife and other sites. The position is full-time and on-site with expected travel to multiple sites.

Formación

• Master of Social Work (MSW) degree required; licensure in Massachusetts preferred (LICSW) or on path to LICSW.
• 3–5 years of experience in direct care and/or care coordination, with trauma survivors a plus.
• Experience in elder care settings and with diverse communities is preferred.

Responsabilidades

• Provide trauma-informed, person-centered coordination of care for CPEAN clients.
• Maintain relationships with community partners and referral sources across Greater Boston and Eastern Massachusetts.
• Perform intake, assessments, and discharge planning using SSPs and trauma-informed tools.
• Lead outreach to referral sources to improve referrals and admission procedures.
• Collaborate with team to address housing needs and coordinate with APS and elder justice initiatives.
• Deliver educational trainings and presentations on elder abuse prevention to various audiences.
• Contribute to documentation policies and post-discharge follow-up procedures.
• Represent CPEAN in meetings and donor events as needed.
