Hebrew SeniorLife in Roslindale, MA, seeks a Supportive Services Social Worker, Intake Specialist to provide trauma-informed, person-centered coordination of care for CPEAN clients.

This full-time, on-site role requires regular travel to other HSL sites across Greater Boston and Eastern MA, coordinating with referral sources and community partners to support elder abuse prevention and housing needs.

Qualifications

• Minimum 3-5 years of direct care and care coordination experience.
• Masters in social work and MA licensure required; LICSW preferred.
• Knowledge of safety planning and trauma-informed care, or willingness to learn.
• Experience delivering trainings (virtual/in-person) is a plus.

Responsibilities

• Provide case management and coordination for CPEAN clients in collaboration with partners.
• Lead elder abuse prevention line and timely follow-up on referrals.
• Perform intake, assessment, and discharge planning using trauma-informed tools.
• Create and monitor Supportive Services Plans and track progress.
• Work with team to assist clients with appointments, benefits, and counseling.
